Life & Long-Term Disability Insurance

Basic Life and AD&D:
Paid by the University, is a benefit for full-time employees working 30 or more hours per week. The life insurance coverage is equal to two (2) times the annual salary.

Additional Voluntary Life Insurance:
Available for the employee to purchase in $10,000 increments, up to 5 times an employee's annual salary. Guarantee Issue for newly eligible employees is $300,000 if under age 70.

*Benefits will reduce 35% at age 65; Additional 15% of original amount at age 70; Additional 15% of original amount at age 75. Benefits terminate at retirement.
*The monthly rate per $1,000 of insurance is .1100

Dependent life insurance, is available in options of $10,000, $20,000 and $50,000, with or without AD&D is also available.  

Dependent Life Insurance

Long-Term Disability Insurance (LTD), paid for by the University, is a benefit that pays if an employee is sick or hurt and unable to work for 6 months, at the end of the 6 months the employee will begin drawing what equals 60% of their base pay.  The benefit will continue as long as the employee is disabled or age 65.  Each employee must be employed six months, before the Long-Term Disability benefit goes into effect. 

Employees may purchase a 3 month buy-up option, which will reduce the waiting period from six months to only three months before LTD benefits can begin.


Help Completing Your Statement of Health

Also known as an Evidence of Insurability, a Statement of Health (SOH) is required if you are electing or increasing life or disability coverage. You can increase already existing voluntary life coverage by up to $20,000 each open enrollment without a Statement of Health (increases over $300,000 will still require SOH).
